Only one form of urticaria has whealing that is sufficiently characteristic to suggest a specific diagnosis. Which one? The initial wheals in cholinergic urticaria are quite different from those in other forms of urticaria. They are small, punctate monomorphic (often referred to as pencil-eraser–sized, ~0.5-cm diameter) with a prominent erythematous flare.
