What is the cause of most chronic urticaria?

In most patients seen in referral centers, chronic urticaria remains unexplained despite extensive workup. However, the two largest subgroups of chronic urticaria patients have lesions, induced from physical stimuli (e.g., heat, pressure, vibratory, and cold) or with an autoimmune basis.
