What is “North Carolina spotless fever”? The tickborne disease caused by Rickettsia rickettsii is usually called Rocky Mountain spotted fever. However, this name is a misnomer, because the disease rarely occurs today in the Rocky Mountain states but is common in the vicinity of North Carolina. Perhaps 20% of cases lack the petechial eruption at presentation. Each year, there are more cases of North Carolina spotless fever than there are of Rocky Mountain spotted fever—hence, the proposed name change.
