What is a primary skin lesion?

It is the initial lesion that has not been altered by trauma, manipulation (scratching, scrubbing), or natural regression over time. Examples include:

Macules
Wheals
Papules
Vesicles
Plaques
Bullae
Patches
Pustules
Nodules
Cysts
