What are common causes of generalized pruritus?

The numerous causes of generalized pruritus are numerous but can be classified as category I, dermatologic origin of pruritus; category II, systemic origin of chronic pruritus; and category III/IV, neurologic and psychiatric/psychosomatic origin of pruritus.
