Do any special considerations exist when performing skin surgery on patients with skin of color? Due to the increased risk of pigmentary alterations, hypertrophic scars, and keloids among black patients, any surgical undertaking should be carefully considered with respect to the risks and benefits of the procedure. Because melanocytes are more sensitive to cold injury than keratinocytes, liquid nitrogen can cause permanent loss of pigmentation, and this is generally more noticeable in darker skin types. Treatment of benign growths, such as warts or seborrheic keratoses, with cryotherapy can result, therefore, in permanent loss of pigment, and this modality must be judiciously implemented.
